Abstract

Introduction: Dystonia is a heterogenous neurological disease, characterized by abnormal muscle contractions, for which standard medical therapy is often inadequate. In this situation surgery is an option. Many different surgical techniques are available in the treatment of dystonia, but evidence for these techniques is more established in inherited and idiopathic isolated dystonia than in acquired dystonia. Objectives: The objective of this master thesis is to review the evidence for treating acquired dystonia with various surgical approaches and to discuss recent advances from the last five years relevant to patient evaluation, treatment selection, and surgical outcomes. Methods: A comprehensive literature search for the usage of surgical approaches in the treatment of acquired dystonia was performed in PubMed/Medline, Embase and Cochrane databases for articles published after the article ‘Phenomenology and classification of dystonia: a consensus update’ of 2013 up until January 2019. Results: In the surgical treatment of acquired dystonia, there are four different avenues that are described in literature. Most literature by far can be found on deep brain stimulation. Stereotactic lesioning is an older and currently less frequently applied technique. Surgical peripheral denervation and intrathecal baclofen have also been routinely described. Conclusion: The evidence for surgical treatments in acquired dystonia is of low quality and quantity and more standardized and specific trials for distinct etiologies in acquired dystonia are needed. Nevertheless, certain conclusions could be drawn regarding treatment selection for prevalent etiologies, such as cerebral palsy and tardive dystonia, and concerning contraindications and adverse events associated with each surgical option.

Abstract

Invasieve soorten kunnen een negatieve invloed hebben op hun omgeving. Bij de introductie in een nieuw gebied hebben zij het voordeel dat mogelijke parasieten en predatoren niet aanwezig zijn of niet zijn aangepast zijn. Dit is de basis van de enemy release-hypothese (ERH) en geeft een mogelijke verklaring waarom invasieve soorten juist zo succesvol zijn. Om deze hypothese te testen zijn parasieten een interessant studieobject aangezien zij mee kunnen worden geïntroduceerd met hun gastheer en zo de lokale fauna negatief beïnvloeden. Twee invasieve vissoorten die Vlaanderen recentelijk hebben bereikt zijn de zwartbekgrondel (Neogobius melanostomus) en de marmergrondel (Proterorhinus semilunaris). Van de zwartbekgrondel wordt aangenomen dat hij in deze regio is geïntroduceerd met het ballastwater van transport-schepen. De introductie van de marmergrondel zou het gevolg zijn van natuurlijke verspreiding via het nieuw gegraven Donau-Main-Rijnkanaal. In deze masterproef wordt een eerste inzicht verkregen in enkele facetten van de succesvolle invasie van beide grondelsoorten. Ten eerste werd de algemene verspreiding in Vlaanderen in kaart gebracht als basis voor verder onderzoek. Ten tweede werd aan de hand van genetische merkers getracht de genetische variatie en oorsprong van de Vlaamse populaties te achterhalen. Als laatste werden de parasieten bekeken. Enerzijds werden de infectiekenmerken berekend en anderzijds werden de parasieten moleculair en morfologisch gedetermineerd. De zwartbekgrondel lijkt zich vanuit de regio rond Antwerpen landinwaarts te verspreiden door-heen de rivieren en kanalen. Drie haplotypes, die elk in een mutatie verschilden, werden ontdekt. Twee haplotypes kwamen overeen met haplotypes bekend van de Zwarte Zee. Er werd een lage genetische variatie gemeten. De marmergrondel bereikte Vlaanderen via de Grensmaas en ver-spreidt zich van daaruit doorheen Vlaanderen. Twee haplotypes, die met zeven mutaties van elkaar verschilden, werden gevonden...

Cicero vs. Mark Antony: ingroup/outgroup formation in Philippics I and III

Abstract

This Master Thesis has carried out an identity analysis of two of the last speeches of Cicero, a Roman orator from the first century BC. These speeches, viz. Philippics I and III, were delivered before the senate and directed against Mark Antony, who had become a leading figure in the Roman Republic after Caesar’s murder. At first, Antony seemed to honor the morals and values of the Republic, but he changed his course on 1 June and behaved like a tyrant in the months following this day. This paper has taken a social constructionist approach to identity, which says that individuals construct identities through linguistic devices. The identity construction of Cicero, the senators, and Mark Antony has been analyzed along three dimensions: 1) sameness/difference, 2) diachronic identity navigation, and 3) agency. 1) is concerned with synchronic variety between social groups. It is a concept from social psychology that people are members of different groups based on, amongst others, shared characteristics. Groups define themselves mostly by their differences with other groups. Thus this paper has looked at the linguistic devices that Cicero uses to create a group consisting of himself and his audience (the ingroup) and a group consisting of enemies (the outgroup). The analysis has shown that Cicero draws on common ground, uses activities that are specific for his ingroup, and uses ingroup reference devices to construct his ingroup and to create unity within it. Moreover, it has shown that he uses negative evaluatives, negative actions, dissociating demonstratives etc. to dissociate his ingroup from Mark Antony. 2) is concerned with diachronic change in identity construction. The diachronic identity navigation of Mark Antony’s identity was analyzed along two parameters. Firstly, Mark Antony’s change of identity of 1 June was sudden, as opposed to gradual. More importantly, the second parameter had a different value in Philippics I and in Philippic III: whereas Mark Antony’s real (good) identity was corrupted by bad advisors in Philippic I, it was uncovered in Philippic III. These different values are an indication of changes in Cicero’s approach to Mark Antony. 3) is concerned with the representation of an individual’s agency. High-agency marking devices present a person as an active hero who is in control. Low-agency marking devices present a person as a passive undergoer, who is not accountable for his actions. This analysis has shown that Cicero avoids expressing agency when his ingroup engages in negative activities and when Mark Antony harms the Republic in Philippic I. On the other hand, he uses high-agency marking devices when Mark Antony harms the Republic in Philippic III. Moreover, Cicero uses agency in the construction of a ‘super-agent’ role for Mark Antony and a victim role for himself and his ingroup in Philippic III. In short, this analysis has shown that the representation of the ingroup does not change between Philippic I and III. More importantly, it has shown that the representation of Mark Antony changes from an unaccountable man led astray by his bad advisors in Philippic I, to a fully responsible, dangerous enemy to the Republic in Philippic III. Thereby it has shed new light on an ongoing debate about the starting date of the conflict of Mark Antony and Cicero, so that it may be argued that the conflict, in terms of enmity and active outgroup construction, started with the Third Philippic.

Abstract

This research analyzes the evaluations of multiple artistic organizations who applied for a working grant from the Flemish Government. These grants can be the primary source of income for some of these organizations, so the importance of a fair evaluation is manifest. Content analysis was used to transform the text of the evaluations into workable data. Analysis shows that the criteria that were used for the evaluations were not applied consistently, and have thus led to an unfair evaluation. The recommended solution to mend the current situation is to redefine the criteria to make them less vague, and to provide a checklist for the reviewers to help them stay on topic.
